BayCare HomeCare is a large home health agency that spans 13 counties in central Florida. When clinician communication issues made it difficult to deliver patient centric-care, leaders developed a plan that identified gaps in the care process and allowed for more effective and meaningful communication between clinicians. In addition to providing an overview of how virtual multidisciplinary rounds were designed and implemented, this session?s presenters will explain how team member push-back was addressed and how attendees can create their own multidisciplinary rounding strategy using virtual tools to break down geographic barriers.
