In 2015, Intermountain deployed its High Reliability initiative to achieve Zero Harm across 22 hospitals, 185 clinics, and all nonclinical business units, training nearly 38,000 caregivers in error prevention techniques. Physicians were also required to take a three-hour error prevention class. After assessing their safety culture progress in 2018, leaders realized that physician adoption was not meeting expectations, with many physicians skeptical of the Zero Harm concept.
