In this session, presenters from OSF Home Health will discuss how the system rose to the challenge of embracing accountable care?particularly around the shift to coordinated care and improved care transitions?while also addressing high costs and low home health referral capture rates among ACO patients. By upending its longtime case management model, OSF Home Health was able to accomplish significant progress across multiple fronts within one year, including significantly improved patient satisfaction performance with sustained upward trends, enhanced nursing continuity, reduced 30-day readmission rates, a decrease in the average number of visits per patient per home health episode and a total multimillion-dollar financial improvement. Attendees will learn how to take inventory of the current cultural beliefs within their organizations and ways to provide new experiences to address those beliefs and improve morale, culture and patient experience outcomes at a lower cost.
