The medical homeā¦patient-centered medical home (PCMH) is a team based health care delivery model led by a physician, PA or NP that provides comprehensive and continuous medical care to patients with the goal of obtaining maximized health outcomes (American College of Physicians) (American Academy of Family Physicians)." Medical Fitness Programs provide a comprehensive integration of medical services not only to the well-bodied but also to the remaining 80% of the population who have underlying medical conditions, 75% of which consists of chronic disease. By addressing each individual's level of fitness, nutritional status, underlying medical morbidity and life stressors, a significant positive impact on our nation's socio-economic expenditure can be realized. In this session, Dr. Robert S. Gotlin, Director of the Medical Fitness Program at the Continuum Center for Health and Healing will present data supporting our nation's $2.8 trillion healthcare expenditure and offer ideology to re-direct our medical interventions toward optimizing well-being.
